Transitional Care

Harmony’s Transitional Care Program (TCP) is a 3-month treatment experience involving group and individual therapy, combined with a sober living environment. Individuals who have completed residential treatment at Harmony or another treatment facility engage in a peer-based community, learning how to apply recently acquired strategies into their new life in recovery. TCP counselors facilitate intensive outpatient groups four days a week.

TCP clients live in comfortable, gender-separate houses. Recovery house managers work with residents to enhance motivation for lifelong recovery by introducing work and volunteer opportunities, sober fun activities, wellness and nutrition classes, relapse prevention techniques, and engagement in a Twelve Step recovery community.

In TCP, clients receive therapeutic services primarily through group work, but they are also assigned an individual therapist who will meet with them on a weekly basis while in treatment. TCP provides program participants with education on a variety of topics to enhance social, coping, and life skills to support the transition into recovery.

Harmony and the YMCA of the Rockies
Harmony TCP clients who are accompanied by staff are able to utilize the YMCA of the Rockies recreational facilities.  Activities include but are not limited to:

  • Indoor swimming pool
  • Miniature golf
  • Rollerskating
  • Indoor/outdoor basketball and volleyball courts
  • Ice skating
  • Nature programs
  • Kickball
  • Capture the flag
