Clinical Case Management

Preparation for continuing care begins when clients enter our program. A clinical case manager is assigned to each client. Weekly meetings between the case manager and client give them the opportunity to plan for care after they leave Harmony. Knowing our clients well allows us to develop a continuing care plan that meets their needs.

Continuing care lies on a spectrum of intensity from self-help groups such as Alcoholics/Narcotics Anonymous to extended care programs that offer clinical programming and sober housing. Clinical case managers meet clients where they are at in terms of which levels of care the client is expressing interest in and other factors that may influence their continuing care plan, such as finances. Clients are encouraged to have set appointments with continuing care providers upon discharge.

A solid network of recovery support is a critical aspect of maintaining sobriety. As clients near the end of their treatment program, we connect them with long-term alumni in their area who can continue the peer support and help them connect with the local recovery community.

Our concern for our clients does not end when they finish residential treatment. Harmony provides ongoing support services for those who have completed our residential program in our aftercare program. Harmony Aftercare is not a 12-Step program but a process group run by a Harmony counselor. Counselors work with clients and their family members to address issues in their recovery. In addition, counselors are able to notice early signs of potential problems and get clients the help they need to prevent a relapse. Harmony Aftercare meets weekly in two locations, one in North Denver and one in Loveland. This program is available at no cost to clients and their family members.
